Facility Use Request

Please review the following before you fill out the form.

COVID Guidelines

June 7, 2020 Until June 30, 2020

For Phase 1: (effective 6-7-20)

  1. (Per the governor’s ruling) “Churches are allowed to meet at 50% capacity” applies to worship services only.
    Internal Groups/Committees of 30 or less may meet in person in the Sanctuary only; social distancing and masks required.
  2. All Groups meeting at the church will be asked to complete a Facility Use form for approval.
  3.  Groups will be directed to clean as they leave. A caddy with cleaning supplies and a checklist will be provided.
  4. Bathroom use is limited to the family bathroom in the foyer area. Cleaning supplies and a checklist will be provided.
  5. Internal groups/committees may schedule to use the church’s Zoom account. (coordinate with the church office on availability).
  6. Groups meeting outside may enter the building to use the family bathroom in the foyer area. Masks should be worn when entering the building.
  7. Only the Sanctuary is available for meetings at this time. No other rooms in the building will be open during Phase 1.
  8. No outside groups at this time. (Requests made prior to shut-down will be honored as we are able to accommodate them)

 

July 1, 2020 until July 31, 2020

Effective July 1, 2020

  3. Any Group/Committee of 12 or less may meet in person in classrooms 1, 3, 25, and 27 (only larger classrooms); social distancing and masks required
  4. Any Group/Committee of 8 or less may meet in person in any classroom (any size classroom); social distancing and masks required.
  5. All Groups meeting at the church will be asked to complete a Facility Use form for approval.
  6. Groups will be directed to clean as they leave. A caddy with cleaning supplies and a checklist will be provided.
  7. Internal groups/committees may schedule to use the church’s Zoom account. (coordinate with the church office on availability).
  8. Groups meeting outside may enter the building to use the family bathroom in the foyer area. Masks should be worn when entering the building.
November 13, 2020 until... we don't know.

Effective November 13, 2020

  1. Any Group/Committee of 50 or less may meet in person in the Sanctuary (150 or less for a religious service); social distancing and masks required.
  2. Any Group/Committee of 20 or less may meet in person in classrooms 1, and 3,  social distancing and masks required.
  3. Any Group/Committee of 18 or less may meet in person in the M.S. and H.S classrooms, and in rooms  25, and 27; social distancing and masks required.
  4. Any Group/Committee of 12 or less may meet in person in any classroom (any size classroom); social distancing and masks required.
  5. All Groups meeting at the church will be asked to complete a Facility Use form for approval.
